Student non transportation form
Thoiuisand Oaks High School Student Non-Transportation Form
This form needs to be signed and given to your coach when the athlete has taken the bus to a game but you wish to take them home early, or in your own vehicle and not take the bus home.
Please fill this form out and give to your head coach of your team PRIOR to the game giving the coach documentation that your daughter will not be taking the bus home.
We need to make sure every player is accounted for and a phone call to the coach is not suffice, this form is needed each time.
