Athletic Clearance.com website instructions:

**Getting your daughter cleared to play sports by TOHS is a process handled by the parent(s).

It is no longer handled by the school. 

Do not drop off any forms to the TOHS Athletic Department, front office, and please do not give forms to any coaches, they do not have the authority to handle these forms any longer.

 

There is a form that your child's physician must complete after her physical.

The form you need to the doctor to fill out is below in a PDF form, please print and take with you to the doctor appointment.

There is typically a fee the doctor will charge to complete the form.

Do not let the doctor mail the completed form to you, rather wait in their office for them to complete the form prior to you leaving.

 

Once the form is completed by the doctor upload the form to the Athletic Clearance.com website.

Here's a link on how to upload and complete once you get into their website, make sure and click the 2020/2021 school year, the form has the wrong year.
